Insurance We Accept
Medi-Cal & Medi-Medi pending enrollment — call for latest status.
What to Bring
- Valid government-issued photo ID
- Insurance card(s), if applicable
- List of current medications including dosages
- Relevant medical records or recent lab/test results
- Referral forms or insurance authorization documents, if required
- Completed new patient forms (if provided in advance)
- Payment for copays, deductibles, or self-pay fees
What to Expect
Please arrive 15–20 minutes early to complete paperwork and check in. Your provider will review your health history and develop a care plan tailored to your needs.
For patients enrolling in the Suboxone Program or chronic pain management, additional documentation may be requested.
